MISSION STATEMENT

It is the policy and law of the State of West Virginia that the practice of

speech-language pathology and audiology is a privilege granted to qualified persons and that, in order to
safeguard the public health, safety and  welfare, protect the public from being misled by
incompetent, unscrupulous and unauthorized persons,
and protect the public from unprofessional conduct by
qualified speech-language pathologists and audiologists,
it is necessary to provide regulatory authority over persons
offering speech-language pathology and audiology services.




Jurisprudence Exam

  ALL inital/first time Audiology and/or Speech-Language Pathology License applicants are REQUIRED to complete the Jurisprudence Exam.  Simply click on the Jurisprudence Exam link above to begin.

ACTIVE/CURRENT LICENSEES WILL RECEIVE ONE (1) HOUR OF ETHICS CONTINUING EDUCATION CREDIT BY TAKING THE JURISPRUDENCE EXAMINATION AND SUBMITTING THE COMPLETION CERTIFICATE WITH THE REQUIRED CE DOCUMENTATION FOR LICENSE RENEWAL.


 

LICENSE APPLICATION SCENERIOS

 

         Below are several different scenarios with an overview of how to apply for a SLP OR AUD license with the WV Speech-Language Pathology (WVBESLPA).  These scenarios do not include instructions for a provisional license (completion of a fellowship year) or an Assistant's registration.  If you have questions, PLEASE do not hesitate to contact our office. 

 

1. If you are ASHA Certified but have never applied for and/or have never been licensed by the WVBESLPA:

a. Complete the “Initial Application for SLP & AUD”(found under the Forms link).  The application must be signed and notarized.  There are also instructions for completing the application under the Forms link.

b. Include a copy of your ASHA certification card

c. Complete the Jurisprudence Exam – required for all new applicants. (Click the link at the top of the page) Print the certificate when finished with the exam and submit with the application.

d. Include the application and licensing fees - Current license period is January 1, 2019 thru December 31, 2020

$300 - January 1 thru June 30, 2019

  $250 - July 1 thru December 31, 2019
  $200 - January 1 thru June 30, 2020
  $150 - July 1 thru December 31, 2020
All SLP and AUD licenses expire December 31st every two years on the even year, i.e., 12/31/2020

2.  If you are NOT ASHA Certified and have never applied for and/or have never been licensed by the WVBESLPA:

a. Please call our office @ 304-473-4289 to discuss the qualification requirements for WV licensure per WV Code §30-32 & WV Code Rule §29-1.

 

3.  If you were previously licensed by the WVBESLPA and your license is expired 1 year or less:

a.  Complete the renewal application via the Online Renewal link. You will be required to enter your SLP license number (SLP-XXXX) and the last four of your social to access the renewal process.  If you do not know your license number, please call 304-473-4289 and we will be glad to assist you. The total fee is $250.00 for license renewal including the late fee.

OR

b.  If you prefer to mail a renewal application with a check; download, print and complete the Renewal Application under the Forms link.  Submit the application with a check for $250.00

c. CONTINUING EDUCATION - Submit 20 hours of approved Continuing Education completed during the last licensing period.  If you renew online, please either fax or email the continuing education documents.  If you mail the application, please submit the continuing education with the application and renewal fees.

 

4.  If you were previously licensed by the WVBESLPA and your license is expired 1 year + 1 day and no more than 5 years:

a.  A license reinstatement is required.  Complete the renewal application via the Online Renewal link.  You will be required to enter your SLP license number (SLP-XXXX) and the last four of your social to access the renewal process.  If you do not know your license number, please call 304-473-4289 and we will be glad to assist you.  The total fee is $275.00

OR

b.  If you prefer to mail a renewal application with a check; download, print and complete the Renewal Application under the Forms link.  Submit the application with a check for $275.00.

c.  CONTINUING EDUCATION - Submit 10 hours of approved Continuing Education for each year your license was inactive or a maximum of 25 hours.  If you renew online, please either fax or email the continuing education documents.  If you mail the application, please submit the continuing education with the application and fees.

 

5.  If you were previously licensed by the WVBESLPA and your license is expired 5 years + 1 day or longer:

            a.  You are required to apply for a new license.  Please follow 1 and/or 2 above.
